To make a payment by mail:
1) Make personal check, money order or cashiers check payable to FAMS
2) Mail your payment to:
Please reference any letter notification received from FAMS for the appropriate P.O. Box to make your payment. If you cannot locate mailed communication, you may call in to our office to obtain it from a FAMS representative at 1-800-399-4639.
Please include your name, address, social security number and FAMS ID number (if available) with your payment.
